What the seat is doing
In a frontal crash the vehicle stops and the occupant does not. Every restraint system exists to take the energy of that unstopped body and spread it out — over as much surface area as possible, over as much time as the structure can buy, and onto the parts of the body most able to survive the load. A child restraint does this differently from an adult belt for one blunt reason: a child's skeleton and proportions are not what the adult belt geometry was designed around, and a small child's head is a much larger fraction of their mass.
That single fact explains most of what follows. Rear-facing works because it lets the entire back of the shell take the load and keeps the head, neck and spine moving together instead of the head being thrown forward on a neck that cannot hold it. A five-point harness works because it puts the load on the shoulders and hips — the strong points — rather than the abdomen. A booster works because it raises a child so the vehicle's own belt crosses the collarbone and the pelvis instead of the throat and the stomach. Each stage is a way of getting the force onto bone.
Which stage a given child should be in, and for how long, is governed by two documents and one body of law: the car seat manual, the vehicle manual, and the law of the state you are in. Requirements vary by state and get amended, and separately, the height and weight limits that matter for safety are printed on the seat itself. This page states none of them, because any number written here would be wrong somewhere and out of date eventually. The errors that show up in inspection after inspection
Technicians who check seats at inspection stations report a small, stable set of mistakes. They are not exotic. Most of them are made by careful parents who read something once.

The pattern worth noticing is that almost every one of these is a detail that seems minor, is invisible once the child is buckled in, and quietly removes a large share of the protection the seat was tested to deliver.
Winter coats, and what else goes under the harness
A bulky coat compresses in a crash. The harness was tightened around the coat and not around the child, so when the material flattens the harness is suddenly loose by exactly the thickness of the puffer, and slack is the thing the whole system is built to eliminate.
The standard approach is simple and unglamorous: coat off, buckle the harness properly against the body, then put the coat on backwards over the buckled child or lay a blanket over the top. The same logic applies to snowsuits, thick fleeces and any aftermarket insert, strap cover, head positioner or seat protector that did not come in the box with the seat. Aftermarket accessories are not part of what the seat was crash-tested with, and many manufacturers state that using them voids the certification.
The everyday version of this test costs nothing: buckle the child in wearing the coat, tighten as usual, then unbuckle without loosening anything, take the coat off, put the child back in, and buckle up. Go in with the seat already installed the way you normally have it, rather than uninstalling it to look tidy — the point is for them to see what your child actually rides in. Bring both manuals if you have them, and bring the child. Expect the appointment to be partly a lesson: a good technician will have you remove the seat and reinstall it yourself while they watch, because the goal is that you can do it correctly next time and after every time the car is cleaned out.
What they will not do is tell you what your state requires you to do, in any binding sense. That is law, it differs by state, and it changes. Read your own state's current requirements, and understand that legal minimums and best practice are not the same thing — the manuals and the seat labels are usually the more protective of the two.
The habits that survive the parking lot
Two things go wrong repeatedly after a perfect install. The first is that the seat gets moved — for a car seat swap, a road trip, a rental, a grandparent's vehicle — and gets reinstalled in a hurry with nobody checking the belt path. Re-run the inch test every single time the seat is moved, including moves you did yourself and were sure about.
The second is that the harness never gets retightened as the child grows and as layers change through the year. The pinch test and the chest clip are a two-second habit per trip and they are where most of the real-world protection is either kept or given away.
And there is one hazard the seat has nothing to do with. A child is never left alone in a parked vehicle, in any weather, for any length of time — vehicle interiors heat far faster than intuition suggests and cracked windows do very little. The habit that catches this is putting something you cannot leave without, a phone or a badge or a bag, in the back seat next to the child, every trip, so that the routine itself does the remembering on the day your routine breaks.

What does my state require?
This page does not say, deliberately. Child restraint requirements are state law, they differ across states in age, weight and height terms, and they get amended, so any figure written on a national page would be wrong somewhere and out of date eventually. How tight does the installation have to be?
The standard check is under an inch of movement front to back and side to side, tested by gripping the seat at the belt path — the place where the belt or the lower anchor strap passes through the seat — rather than at the top of the shell. The top will always move somewhat, and testing there is the most common reason people believe a loose install is tight. Redo this test every time the seat is moved between vehicles or reinstalled after cleaning the car.
Can my child wear a winter coat in the car seat?
Not under the harness.
