What the UV index is
The UV index is a forecast of how strong ultraviolet radiation will be at the surface at a given place and time, on an open-ended scale where higher means faster damage and shorter time to a burn. It is issued daily and it varies with the season, the time of day, latitude, altitude, cloud cover and the state of the ozone layer overhead. The categories carry advice — the higher end recommends progressively more protection and more caution about midday exposure — and the sensible way to use the index is as a signal to plan, not as a threshold to be argued with.
What it does not do is describe your skin. Two people standing in the same forecast get very different results, and personal history matters more than any published number: a tendency to burn rather than tan, a lot of moles, freckling, light eyes or hair, a family history of skin cancer, past bad burns, or medication that increases sensitivity. If any of that applies to you, take the conservative reading of the day's forecast and take whatever your clinician has told you over anything general.

This is why an SPF number on its own is an incomplete answer. A high SPF with no broad spectrum claim can keep you from turning red while doing less about the rest of it, and "did not burn" is a poor measure of a day's exposure. Look for broad spectrum, and look for water resistance if you will be swimming or sweating — the label states the time period the water resistance was tested for, and it is a tested duration, not a promise of the afternoon.
Cumulative, reflected, and on cloudy days
Damage adds up. A burn is not delivered by one long exposure so much as by the total the skin took that day, which is why the walk to the car, the twenty minutes at the bus stop, the hour at a kid's game and the drive home stack into an evening of red skin that nobody can account for. People who never "go out in the sun" accumulate substantial exposure this way.
Reflection adds to it from below. Snow reflects a large fraction of what hits it, which is why ski days burn the underside of the chin and the nostrils and why eye protection on snow is not optional. Water and wet sand reflect meaningfully as well, and so does light-colored concrete. Being under a beach umbrella cuts the direct sun and does very little about the light coming up off the sand, which is a common way to get burned while sitting in shade all day.
Altitude increases intensity, and every thousand feet up adds to it — a hike at elevation on a cool day is a classic sunburn, because the temperature gives no warning. Cool weather is the general trap: skin damage is caused by ultraviolet light, not by heat, and an overcast, breezy day at the coast can burn people who felt cold the entire time. The amount, which is where the label goes wrong
SPF is measured in a laboratory using a standardized quantity of product per area of skin, and it is a thicker layer than almost anyone applies. Studies of real-world use consistently find people applying a fraction of the tested amount, and the protection you get falls off faster than proportionally. The practical consequence is that the number on a bottle is close to a best case, and the way to get near it is to use more product than feels normal.
Useful rules of thumb, not precise measures: roughly a shot-glass-sized amount to cover an adult body in swimwear, and roughly a teaspoon-sized amount for the face, neck and ears together. Most people apply perhaps a third of that. Two passes, waiting a moment between them, gets closer to the intended layer than one thin pass and is easier than trying to estimate.

Reapplication is the whole game
This is the step people skip, and skipping it is the reason applied sunscreen underperforms its label more often than any other factor. Sunscreen comes off. It rubs off on a towel, a shirt collar, a car seat and a backpack strap; it sweats off; it washes off; and it degrades with exposure. The general guidance is to reapply roughly every couple of hours during sustained exposure and immediately after swimming, heavy sweating or towel-drying, regardless of what the water resistance claim on the bottle says — the claim is a tested duration, not an exemption.
Two practical notes. Applying a little before going out lets a film form evenly rather than being immediately smeared, and it costs nothing to do it while you are still inside. And check the expiration date: sunscreen has one, products degrade faster if the bottle spent the summer in a hot car, and a three-year-old tube found in a beach bag is not doing what the label says.
Shade and clothing beat sunscreen for long days
For a sustained day outdoors — a hike, a shift on a job site, a whole afternoon at a game — physical barriers outperform anything you rub on, because they do not need reapplying, do not sweat off, and do not depend on whether you got an even layer.

People who do not get to choose
Outdoor workers accumulate more exposure in a season than most people do in years, and the protection has to be built into the job rather than remembered: a brimmed hard hat attachment, long sleeves, sunglasses, product kept on the truck and reapplied at breaks, and shade at the rest point. Two other groups deserve a note. People taking medications that increase photosensitivity can burn from exposure that would previously have been unremarkable, and the way to find out is to ask the pharmacist rather than to discover it.
