Where 150 millilitres per kilogram comes from

Infant energy requirements in the first months are commonly put at around 100 kilocalories per kilogram of body weight per day. Standard infant formula and mature breast milk both supply roughly 20 kilocalories per fluid ounce, or about 67 per 100 millilitres. Divide one by the other and you get approximately 150 millilitres per kilogram per day, which is the figure that appears in most references. The 120 to 180 spread around it reflects the ordinary range of individual requirement.

US sources usually state the same relationship in customary units as about 2 to 2.5 fluid ounces per pound of body weight per day, with a practical ceiling of around 32 fluid ounces regardless of weight. These are the same guidance expressed twice, and small discrepancies between the two forms are rounding rather than disagreement.

Why the first week is calculated differently

A newborn stomach holds a very small volume and expands quickly over the first days. The commonly published progression runs from roughly a teaspoon or two on day one to around one and a half to two and a half fluid ounces by the end of the first week. Applying a per-kilogram daily total during that period produces a number far larger than the stomach can hold, which is why the first-week table exists separately and why the pattern in those days is small feeds very often rather than fewer larger ones.

Weight loss over those first days is expected. Losing up to about 7 to 10 percent of birth weight and regaining it by around ten to fourteen days is the normal course, and it is one of the things checked at the early weight visits.

Breast milk does not scale with weight

This surprises people who have been using the per-kilogram figure. Measured intake in exclusively breastfed infants sits at roughly 750 to 850 millilitres a day and stays close to that from about one month through six, even as the baby doubles in weight. Milk composition shifts over that period instead — fat and energy density change — so the volume does not have to.

The practical consequence is that a breastfeeding parent who tries to hit a rising per-kilogram target with expressed milk is chasing a number that does not apply. Directly fed infants are assessed by output and weight, not by volume, which is a more reliable method than measuring would be even if measuring were easy.

Why there is no scoop conversion on this page

Every version of this calculator on the wider internet seems to include one, and it is the single most dangerous thing such a page can do. Powdered formulas differ in scoop size, in the water volume each scoop is made up to, and in energy density between standard, preterm, hydrolysed and specialist products. Concentrated liquid and ready-to-feed formats do not use scoops at all and are not diluted the same way. A conversion that is correct for one tin is wrong for the next, and the error does not announce itself — the bottle looks the same.

The consequences run in both directions and both are serious. Formula made too strong loads the kidneys and can dehydrate an infant; made too weak it underfeeds and, taken far enough, causes hyponatraemia. There is exactly one correct source for the ratio, which is the table printed on the container in front of you together with the scoop that came inside it. If that label is unclear, or the product has changed, ask a pharmacist or the paediatric practice before the next feed rather than estimating.

Questions people ask

My baby takes much more than the calculated amount. Is that a problem?

Not on its own. The published range runs from 120 to 180 millilitres per kilogram, so a baby at the top of it takes half again as much as one at the bottom, and both are ordinary. What is worth checking is whether every cry is being read as hunger, and whether the teat flow is fast enough that the baby finishes before the fullness cues have time to register — a feed that takes under about ten minutes is often a flow issue rather than an appetite one. If intake is consistently high alongside frequent vomiting or weight gain well outside the usual pattern, that is a conversation for a paediatric visit.

How do I know if a breastfed baby is getting enough?

By what comes out and by weight, not by volume. The markers usually quoted are around six or more wet nappies a day after the first few days, regular stools, a baby who is alert when awake and settles after most feeds, and steady weight gain on the practice scale. Direct feeding cannot be measured and does not need to be. Where any of those markers is not there, or feeding is painful or persistently difficult, a lactation consultant or the paediatric practice can assess a feed directly, which tells you far more than a number would.

Should I wake a baby to feed?

In the early weeks the usual advice is yes, if more than about four hours have passed, until birth weight is regained and weight gain is established. After that, a thriving baby who sleeps a longer stretch is generally left to. The threshold varies with the individual baby and the guidance can be different for a baby born preterm, small for gestational age, or with jaundice, all of whom may be on a specific feeding schedule. Whether it applies is a question for the clinician following the baby rather than a rule to read off a page.

When does formula intake start to fall?

Once solid food becomes a meaningful part of intake, usually from around six months. Milk stays the main source of nutrition through most of the first year, but the daily volume drifts down as solids increase, from somewhere in the mid-twenties of fluid ounces at six to eight months towards the high teens or low twenties near a year. The pattern varies a great deal between babies, and the introduction and progression of solids — timing, textures, allergens — is guidance to get from a paediatric visit rather than from a volume calculator.
