What the rule actually interrupts
Every twenty minutes, look at something at least twenty feet away for twenty seconds. Two separate things are going on. The first is accommodation: holding focus at a fixed near distance keeps the ciliary muscle contracted, and looking into the distance relaxes it. The second, and probably the more important, is blinking. Blink rate at a screen falls to roughly half its baseline, and incomplete blinks become more common, so the tear film has longer to thin and break up between refreshes. That is the mechanism behind the dryness and the blur that clears for a second when you blink hard.
The numbers themselves are a memory aid. No trial established twenty minutes as better than fifteen or thirty, and the twenty feet is a proxy for far enough that accommodation is effectively relaxed, which happens well before that. In practice, looking away several times an hour accomplishes the same thing without a timer.
Eight hours is twenty-four breaks and eight minutes
Divide 480 minutes by 20 and you get 24 breaks. At twenty seconds each that is 8 minutes, about 1.7 percent of the day. The cost is trivial and people still do not do it, because the obstacle is not the time, it is the interruption of concentration.
Which is why the block schedule exists. Putting the long breaks at block boundaries means most of them coincide with a natural stopping point, and inside a block the micro-breaks can attach themselves to whatever already interrupts you — a build running, a page loading, a message arriving. A 50-minute block contains two micro-breaks. You do not need an alarm for two things.
Distance, height and text size
Around arm's length, 20 to 28 inches, is the common monitor recommendation. Closer means more accommodative effort and a larger screen area in the visual field, which drops blink rate further. If moving the monitor back makes the text hard to read, the answer is bigger text, not a closer screen — squinting is its own source of fatigue and it also narrows the palpebral aperture, which is not the kind of blinking that helps.
Height matters for a reason that is not obvious. Screens placed so the top edge sits slightly below eye level produce a downward gaze, and a downward gaze means the upper lid covers more of the eye, reducing the exposed ocular surface and slowing evaporation. Laptops used flat on a desk are far too low for this, which is the argument for a stand and a separate keyboard.
Contrast, not brightness
The usual instinct is to turn the screen down. Often the problem is the difference between the screen and everything around it. A bright display in a dark room forces repeated adaptation across a large luminance gap; a bright window behind the monitor makes the screen look dim and pulls you toward it. Matching the two roughly — a lamp on in the evening, blinds against direct sun, the desk turned so the window is to the side — does more than adjusting either one alone.
Glare is a related and separate issue. A reflection of a light source in the panel competes directly with the content and is worth removing by moving the source or the screen rather than compensating with brightness.
When the checklist keeps coming back positive
Dryness that persists for weeks after the schedule and the setup have been fixed usually has a cause the schedule cannot reach. Meibomian gland dysfunction, where the oil layer of the tear film is deficient, is the most common; reduced aqueous tear production is next; contact lens wear, rosacea, and drugs with anticholinergic effects such as antihistamines, some antidepressants and isotretinoin all contribute. Preserved artificial tears used many times a day can themselves become an irritant, which is an argument for preservative-free drops if you are using them frequently, and a better argument for finding out what is causing it. An optometrist or ophthalmologist can assess tear film break-up time and lid margin condition in a short appointment, which settles a question that no amount of adjusting your break schedule will.

Do I need to set a timer for every twenty minutes?
No, and for a lot of people the alarm is worse than the problem, because breaking concentration on a schedule has its own cost. The mechanism cares about not holding one focal distance for an hour at a stretch; it does not care about precision. Looking out of the window whenever you finish a task, send a message or wait for something to load hits the same target. The block boundaries in the schedule above are the ones worth being deliberate about, because those are also when you should stand up.
Do blue light glasses help?
The evidence says no, at least for eye strain. A 2023 Cochrane systematic review pooled randomised trials of blue-light-filtering lenses and found no clinically meaningful effect on visual fatigue during computer use, and no evidence of protection for the retina. The discomfort from screen work is largely about blink rate and sustained near focus, neither of which a filter changes. Evening light exposure and sleep timing is a separate and better-supported question, and if that is the concern, reducing overall screen brightness at night addresses it more directly than a lens tint.
Will screen time damage my eyes permanently?
There is no evidence that ordinary screen use damages the retina or causes permanent vision loss. Discomfort from screen work is transient and resolves with rest. The one genuine long-term association is between near work in childhood and myopia development, and even there the strongest and most consistently replicated protective factor is time spent outdoors rather than the specific type of near work. Reading a book close up carries similar associations to a screen. So: screens cause discomfort, not damage, and 20-20-20 addresses the discomfort.
Are eye exercises or eye massage worth doing?
Rolling the eyes around does not improve refractive error and there is no evidence it does anything for acuity. Alternating focus between near and far is a different matter — that is the same mechanism as the 20-20-20 rule, and it works for the same reason. Closing the eyes for a few seconds lets the tear film re-form and is genuinely useful. Rubbing the eyes hard is worth avoiding: it irritates the cornea, and in anyone with keratoconus or after eye surgery it is specifically discouraged.
How many ticked items should send me to a clinician?
There is no validated cut-off, because this is not a validated instrument. It is a list of symptoms commonly reported in screen work, assembled to prompt a question rather than to score one. The useful rule is about persistence rather than count: symptoms lasting more than a few weeks despite better breaks and a better setup, or interfering with work, deserve an examination. Separately, and regardless of anything on this list, sudden vision change, pain in one eye, haloes around lights or an increase in floaters need to be seen the same day.
